SMP INKED

This document provides additional information to what you have already received prior to your treatment day, including the Client Personal Medical Record and ANY information IF provided during your consultation and on our website. This information is designed to help you make an informed decision about undergoing Scalp Micropigmentation (SMP) treatment.

Please ensure you have read and understood this form, the aforementioned documents, and the information provided during your consultation. Only sign the attached Personal Medical Record at your first appointment if you are comfortable proceeding with SMP treatment at SMP INKED. You have the right to change your mind at any time before beginning treatment, even after you have signed the form.

Key Points:

• SMP is an elective procedure and is not medically necessary.
• SMP involves a tattooing technique, where pigments are applied just below the skin. Like traditional tattoos, these markings can be difficult to remove and removal success cannot be guaranteed.
• The pigments used in SMP are made with natural ingredients (carbon and iron oxide).
• Some discomfort or mild pain may be experienced during the procedure.
• As with any tattooing procedure, there is a risk of infection, although serious infections are very rare.
• Clients with a history of skin cancer, sun-damaged skin, or other relevant medical conditions should consult their physician before undergoing SMP. We may require a signed certificate from your physician.
• Compliance with aftercare instructions and attendance at follow-up appointments are crucial for optimal results. MUST BE FOLLOWED.
• Any tampering with the SMP procedure, including failure to attend necessary follow-up sessions, will void SMP INKED's guarantee against potential fading.

Cancellations, Payments, & Complaints:

You will be notified of your treatment appointment time and date. Please confirm your attendance upon receipt of this notification. Failure to confirm may result in your appointment being reallocated to another client. All SMP INKED practitioners are trained, approved, licensed, and insured to perform SMP.

If you are unable to attend your appointment, please provide 48 hours' notice so we can reallocate your appointment. Cancellations or reschedules within 48 hours will incur a $250 rebooking fee plus the full session payment to be rescheduled. Please arrive at least 15 minutes prior to your appointment to complete necessary paperwork. Late arrivals may result in reduced treatment time or forfeiting the appointment. SMP INKED may need to cancel or postpone appointments at short notice; we will make every effort to inform you in advance. 

Compensation is not offered for postponed appointments.

Deposits and Payments:

SMP INKED may request a non-refundable deposit to secure your appointment. This deposit will be deducted from your treatment cost. Canceling or missing an appointment will forfeit the deposit. If you reschedule with 48 hours' notice, the deposit will be applied to the new appointment. Cancellations or reschedules within 48 hours will incur a $250 rebooking fee plus the full session payment to be rescheduled. Accepted payment methods: cash, debit/credit cards (3% fee), and certified bank checks. 50% of the treatment cost is payable before the first session. Quotes are valid for one year.

Treatment Suitability:

We will assess whether the treatment is suitable and likely to be successful before proceeding. If unsuitable, we will explain why.

Liability:

SMP INKED is not liable for economic loss or other special, indirect, or consequential loss or damage. It is the client’s responsibility to provide all relevant medical details. SMP INKED is not liable for damages resulting from the client’s failure to disclose such details. Clients must follow all care instructions provided.

Guarantee:

SMP INKED provides a satisfaction guarantee against unforeseen fading or lightening for 12 months from the initial treatment date. This guarantee is valid only after completing all of the initially quoted sessions. Changes to the agreed style, including hairline repositioning, density increase, or darker color, are chargeable. Laser removal costs are the client's responsibility. NO REFUNDS for work performed.

Acknowledgments:

I confirm that I have read and understood the information provided. I acknowledge that SMP treatment is a permanent change and there is no guarantee of removal. I understand the treatment's effectiveness varies, and maintenance may be required. I will inform SMP INKED of any skin or medical conditions and any medications. I understand the post-treatment care requirements and will follow all instructions. I consent to photographs for treatment purposes and marketing. I opt-in to receive SMS text messages from SMP INKED. I consent to the storage and processing of my personal data by SMP INKED.
